Directions for Filling Out Questionnaire:
1. Click on “here” at the top of this page to access the questionnaire.
2. Click on “open” for questionnaire to become visible.
3. Read instructions carefully and proceed in filling out your answers for questionnaire. Do not analyze questions too much. Just answer the best you can for yourself.
4. Save questionnaire as a document on your desktop. Save it under your own name. Emily will change the title of your document to a randomly selected numerical title in order to protect your anonymity. This will be done before the document is ever accessed.
5. Once you have saved your file go to your email.
6. Attach your document to an email addressed to: emilystone@forhopeandhealing.com
