Emily’s research is for her work on a Ph.D. in Practical Theology and Religious Studies at the University of Wales. Your responses to the questionnaire are anonymous and confidential. You are under no obligation to participate in this research.
Directions for Filling Out Questionnaire:

3. Read instructions carefully and proceed in filling out your answers for questionnaire. Do not analyze questions too much. Just answer the best you can for yourself.
4. Save questionnaire as a document on your desktop. Save it under your own name. Emily will change the title of your document to a randomly selected numerical title in order to protect your anonymity. This will be done before the document is ever accessed.
